Finding health-care directive forms

A reader helpfully points out that the Minnesota Health Department link we provided in Sunday's article by Jennifer Imsande isn't the easiest way to find health-care directive forms on the Web. Indeed, the Health Department actually links to a different site, operated by the Minnesota Board on Aging, though it has the virtue of first taking you through a series of frequently asked questions about health-care directives.
