Beloved husband, dad, grandpa, brother, uncle, cousin, passed away Saturday, July 12, 2003 at St. Mary's Hospital, Rochester after suffering a brief illness. Survivors include wife, Cora-May, Winona; 5 children, Robert (Criss) Polachek, Winona, David (Cheryl) Polachek, Winona, Ann Polachek, St. Paul, Mary B. (Lance) King, Moundsview and Thomas Polachek, St. Paul; 7 grandchildren; 2 brothers, Ralph (Jean) Polachek, Chicago and Herb (Anne) Polachek, Golden Valley; nephew, Daniel (Lisa) Polachek, St. Paul; along with other cousins, nephews and nieces. Visitation will be at Central United Methodist Church, Winona, at 1:00 PM with services starting at 2:00 PM on Wed., July 16th with Rev. Mary Hurmence, officiating.
