Survived by children, Teri Ballweber (Andrew), Dana Slavik, Brenda Slavik, Matthew Slavik, Mark Slavik- Augustson & his father Chuck; grandchildren, Nathan, Ryan, Kaitlyn, Tianna; father, David Turbitt; other relatives and many friends. Mass of Resurrection 11 am Saturday at the Church of the Epiphany, 111th & Hanson Blvd NW, Coon Rapids. Visitation one hour prior to Mass at church and 5-8 pm Friday, with 7:45 pm prayers at the Washburn-McReavy Seman Chapel, 1827 Coon Rapids Blvd., Coon Rapids. Interment Morningside Memorial Gardens.
