Survived by wife, Sara; father, Chuck (Mary) Simon; step-father, Larry Domek; sisters and brothers, Heidi (Casey) O'Neill, Jen (Lance) Crooks, Matt Underwood, Madison Domek, Calvin (Whitney) Simon, Tony Domek; grandma, Beatrice Simon; many aunts, uncles, nieces, nephews, cousins and friends. Preceded by mother, Mary Domek; grandparents, William Simon, Alvin and Dorothy Theis; uncle, Dave Theis; aunts, Cara Norby and Maria Domek; special friends, "Houff", "Fungo" and Paul.
