A house fire that killed a Minneapolis woman is being attributed to a hot pan left on a countertop.

The combination ignited a smoldering fire that killed resident Kelly Ann Dugan at the house in the 4500 block of Minnehaha Avenue S., the Fire Department said Tuesday.

Dugan was found in her bungalow-style home soon after firefighters arrived about 10:15 p.m. She was pronounced dead shortly after 11 p.m. from smoke inhalation. A dog also died.

City officials said firefighters just wrapped up the most active June in six years and have dealt with about a fire a day so far in July.
