A vehicle hoisted on a forklift in Barron County, Wis., fell and fatally crushed a man, authorities said Tuesday.
The Sheriff's Office was alerted Monday evening to the incident east of Haugen on residential farm property along 28th Avenue.
Patrick J. Uchytil, 50, was declared dead at the scene.
"A forklift was being used to lift a vehicle up to have some work done, and the vehicle slipped off and landed on him," a statement from the Sheriff's Office read.
An online obituary for Uchytil said he performed for more than 20 years at Ceska Opera House in Haugen.
