ID'd: Employee fatally hit by truck inside Mankato tire store's shop

A truck struck and killed an employee inside a tire store's shop in Mankato, authorities said.

The incident occurred about 3:25 p.m. Monday at Tire Associates at 305 W. Lundin Blvd. in an industrial area on the north side of the city, according to police.

Police said Brady J. Schaffer, age 35, of Mankato was given first aid immediately by people on the scene and then by arriving emergency personnel. Authorities have yet to release any details about what led to the death.

The truck, owned by Leavenworth Silage of Sleepy Eye, Minn., was driven by Jason Kuelbs, 26, police added.

Minnesota OSHA will investigate the workplace death to "determine the cause of the accident and determine whether existing OSHA standards were violated to help avoid a recurrence of similar accidents," said James Honerman, a spokesman for the state agency.

The Mankato location of Tire Associates is the business' headquarters. It also has outlets in Alexandria, Minn.; Cedar Rapids and Sioux City, Iowa; and Baraboo, Wis.

In 2010, Tire Associates paid a $420 penalty for two safety violations at this site. One involved railing for an elevated floor and the other addressing a barrier guard on a hydraulic press.
