1 The cover for the main drain of the pool was not securely attached and had been removed at the time of the injury.
2 The water was so cloudy that the main drain was not visible.
3 The main drain outlet wasn't covered by grating that could not be removed by people in the pool. The grating had been removed by pool users the day of the injury.
4 The wading pool and equipment were not operated and maintained in working condition. The main drain cover of the pool had been attached with improper fasteners and a worn mounting ring, which is why it was removed by people using the pool.
5 No one was available to respond to the unsafe and unsanitary conditions on the day of the injury. Though pool staff members were notified of the loose drain cover, no action was taken to close the pool. Pool staff members were not properly trained.
6 The pool's operation manual did not include installation instructions for the wading pool drain cover.
7 The club did not maintain a record of the pool's operation and maintenance.
