A story on Page B1 Thursday about a sex offender, Thomas Duvall, incorrectly described the role of the state Department of Human Services in his case. The department did not propose that Duvall be released from the Minnesota Sex Offender Program.

A photo caption on Page B3 Wednesday misidentified a Hopkins police officer helping kids fish during the annual Cops N Kids outing on Lake Minnetonka. He is officer Greg Pilon.

A story on Page D1 Friday inaccurately stated that HealthPartners lost 20,000 members in 2012 in its individual health insurance market. That figure includes the group market and other lines of business.
