HonoringChoices.org. The website offers information, videos and documents to help you complete an advance-care directive and designate a health care agent. Forms are available in English, Russian, Spanish, Hmong and Somali.

Any doctor's office. Ask for Honoring Choices information and forms.

Your doctor. Consider talking with your physician or a nurse or a social worker about filling out an advanced-care directive.
