Two big losses of patient info in 2011

In July, a laptop computer with private information on 14,000 Fairview patients and 2,800 patients at North Memorial Medical Center was stolen from a car.

In April, Fairview reported that a box containing information on 1,200 patients was lost during an office relocation. The box has never been found.
