Chart: Preventable Errors in Minnesota Hospitals

The following is a searchable list of hospital errors reported in Minnesota since 2010. Hospitals are required to report 28 different "adverse events" such as operations on the wrong patient. They are often referred to as "never" events because they are considered preventable if appropriate safeguards are used. The raw numbers of errors are not necessarily an indication of hospitals with more safety problems. Larger hospitals generally report more errors. A higher number also could reflect a particular hospital's diligence to report borderline incidents.
