Anoka County is asking residents to identify the most important community health issues facing the county.

The survey, by Community Health and Educational Services, is part of the county's five-year community health assessment and plan, which is required under state law.

Residents are asked about tobacco use, physical activity, nutrition, safety, environment, maternal health, mental health, infectious and chronic diseases, availability of health services, and economic factors affecting health.

The survey can be completed either online in English or Spanish at www.AnokaCounty.us/healthsurvey or on paper. Paper copies in multiple languages are available at county license centers, service centers, libraries, city offices and senior centers.

The survey is anonymous.

As part of the five-year assessment, county staff members also will conduct focus groups and interviews with various groups.
