Most large airlines have medical fares to be used in medical emergencies or hospice situations. The fares allow last-minute booking and other flexibilities to your ticket provided the airline can verify the situation. They must be booked by phone only. Thanks to new Department of Transportation regulations, you can put any air reservation on hold for 24 hours without paying for it. United Airlines also offers something called "fare lock" that allows you to put an airfare on hold for longer periods, on select flights, for a fee.
