The Affordable Care Act requires insurers to spend at least a certain share of the premiums they collect on direct health care or else offer rebates to customers. Here are the companies paying rebates in Minnesota:

HealthPartners Inc.

Headquarters: Bloomington

Total rebate paid: $6.8 million

Average per family: $163, for large employer plans

Requirement: 85 cents per premium dollar collected

Spent: 83.7 cents

CIGNA (Connecticut General Life Insurance Co.)

Headquarters: Bloomfield, Conn.

Total rebate paid: $1.66 million

Average per family: $1,335, large employer plans

Requirement: 85 cents per premium dollar

Spent: 57.4 cents

American Family Mutual Insurance Co.

Headquarters: Madison, Wis.

Total rebate paid: $205,648

Average per family: $147, for individual plans

Requirement: 80 cents per premium dollar

Spent: 77 cents

Preferred One Insurance

Headquarters: Golden Valley

Total rebate paid: $148,567

Average per family: $50, individual plans

Requirement: 80 cents per premium dollar

Spent: 78.4 cents

Time Insurance Co. (Assurant Health)

Total rebate paid: $140,276
